If your child is between the ages of 0 - 19:
- Simply call 1-888-418-6062. Request our Application for Assistance.
- We will send an enrollment packet with instructions and the necessary forms, including a diagnosis verification to be signed by a medical professional.
- Once CCFA receives the completed application and signed diagnosis verification the first check is sent.
- Applications must be renewed every 6 months for continued assistance.
