Q. I called and left a message. Will my message be received and processed? |

A. Yes. Messages are checked and processed two to three times daily. Please make sure your message and ID number is clearly stated. |

Q. When do I renew my application? |

A. Enrolled patients must renew every 6 months. |

Q. Can I renew my application over the phone? |

A. No. We have to have a hard copy. |

Q. Can I fax in applications or renewals? |

A. First time application must be mailed back. Renewals may be faxed if completely filled out; however, we would rather have the original form. |

Q. When will I get my check? |

A. Checks are mailed out on Friday of each week. After that, it depends on the postal service and how fast/slow they deliver your mail. |

Q. When should I call to request a check? |
|
|
A. Once a month. It doesn't matter what day you call as long as it has been a month since the last check was received or a month since you called. |

Q. Why is just my child's name on the check? |
|

A. Checks are made out to the patients. You may request to have you name added to the check. |

Q. Can you direct deposit my check? |

A. No. |
