Q. I called and left a message. Will my message be received and processed?
A. Yes. Messages are checked and processed two to three times daily. Please make sure your message and ID number is clearly stated.
Q. When do I renew my application?
A. Enrolled patients must renew every 6 months.
Q. Can I renew my application over the phone?
A. No. We have to have a hard copy.
Q. Can I fax in applications or renewals?
A. First time application must be mailed back. Renewals may be faxed if completely filled out; however, we would rather have the original form.
Q. When will I get my check?
A. Checks are mailed out on Friday of each week. After that, it depends on the postal service and how fast/slow they deliver your mail.
Q. When should I call to request a check?
A. Once a month. It doesn't matter what day you call as long as it has been a month since the last check was received or a month since you called.
Q. Why is just my child's name on the check?
A. Checks are made out to the patients. You may request to have you name added to the check.
Q. Can you direct deposit my check?
A. No.
